FollowMyHealth is our online patient portal. It allows you to conveniently manage your personal healthcare, securely communicate with your healthcare provider, and more.
With FollowMyHealth, you can:
- Send secure messages to your providers
- Order prescription refills
- Request, reschedule, or cancel appointments
- View lab results
- Look up vital signs, prescriptions, immunization history, and more
- Receive reminders for upcoming visits
How do I sign up?
Signing up for FollowMyHealth is quick and easy. Any member of our front-desk staff can get you signed up. You’ll just need to provide your name, birthday, and email address. You’ll receive an email invitation with instructions for activating your account online.
Note: Because photo identification is required for security purposes, patients cannot sign up online or over the phone.
How do I access FollowMyHealth?
As part of the activation process, you’ll need to set up a FollowMyHealth login and password, or verify your identity using your Facebook, Google, Yahoo, or Windows Live ID account.
Note: FollowMyHealth does NOT allow these systems to access your medical records.
Once you’ve activated your account, you can access FollowMyHealth by logging in aasthma.followmyhealth.com. If you want to use FollowMyHealth on a mobile phone or tablet, download the FollowMyHealth app from the iTunes Store or the Google Play Store.
